CSB Investigates Fatal Horizon Biofuels Explosion in Nebraska

The U.S. Chemical Safety and Hazard Investigation Board (CSB) is investigating the deadly July 29, 2025 explosion and fire at Horizon Biofuels in Fremont, Nebraska, which killed an operator and his two young daughters, ages 8 and 12. Preliminary evidence points to a combustible wood dust explosion, a known and preventable industrial hazard.

The blast caused extensive structural damage, fires, and smoldering combustible material, with emergency responders unable to recover the victims until the following day. The CSB is examining the facility’s design, dust handling and control systems, operating procedures, and regulatory compliance, as well as interviewing employees, management, and first responders. Investigators are also reviewing prior audits, industry guidance on wood dust safety, and post-incident equipment conditions.

CSB Chair Steve Owens stressed the need to identify root causes to prevent future tragedies, while Board Member Sylvia Johnson emphasized the commitment to a thorough, independent investigation. The facility remains unsafe, and officials advise the public to maintain a safe distance.
